Do I draw attention to the fact that he wasn’t asked to complete any of the mental state cognition or memory tests? It seems wrong that it states he can when it wasn’t covered.
Also, most of the daily living sections are scored 0 but the assessor refers to Giacomo needing prompts from me. Would this be worth pointing out and backing up with diary and evidence from school?

If no tests were done then I would point this out, although their actual value is debatable.
Were they talking about prompting during the assessment or in regard to the PIP activities?
