Thank you for responding and the advice.
Someone phoned up for me and said it would be too overwhelming for me, and I've already suffered so much stress and illness already with this process and tribunal. They said they would ask for a paper based review instead.
I got a text message early last week to say they still want to proceed with an assessment and have booked a telephone assessment for the middle of this upcoming week.
It's taken me nearly a week to be able to sit and ask on here because I'm so stressed to the point of feeling sick with it all, on top of everything else. I'm signed off work at the moment because I'm so burned out and can't focus or concentrate because of extreme fatigue and brain fog.
I will see if someone from a local support charity can phone them to explain the situation and see if they can defer it or put their questions on paper.

I wish I hadn't informed them, but I was advised to do so and made sure to ask if this would mean another assessment. This makes the whole situation worse because I feel I've put myself in this situation and stress when I shouldn't have.

They said they would request a paper based review, but I got a message at the beginning of last week to say I've now been booked a telephone assessment for the middle of this upcoming week. I'm so worried if someone phones up for me again they'll say I've refused and stop my current PIP and I'll have to start this entire lengthy process again and have to go again to a tribunal.

Thank you for the advice. I managed to find someone that could phone up and explain the situation a few weeks ago, and they said they would request a paper review. I got a message last week to say they have now booked me a telephone assessment towards the end of this week. To me that is worse than a video assessment.
It is not right they don't have an email address and you have to phone them. I'm totally lost what to do now, because they have booked it so soon and I have no one that can phone up for me.

Thank you for the comments and advice. I replied to them individually a few days ago but my comments are not showing so I'll try and reply here.
I managed to find someone that could phone for me and explain the process was so stressful and how unwell it all made me before. I am at a loss what to do now because they have already refused a paper review. And I will need to see if anyone from a local disability charity can telephone for me because I can't phone myself.

If you cannot find someone to help you make the call - you could write to the DWP and inform them that you are too unwell to take part in the assessment and say once again that a reasonable adjustment is to make a paper-based decision. Now there is a risk that they may say you are refusing to cooperate and they may cancel your benefit. If they do - you immediately need to appeal. However, they may be reasonable.
One potential issue they are likely to raise is that you work. (I know you are currently off sick).It's not that you can't work and claim PIP. They are likely to assume that if you can work you have contact with other people in the course of your work and therefore you could take part in a telephone assessment. I don't know what you do (and I don't need to), but if this issue is raised, you need to say that it is not the same and the additional stress caused a negative impact on your health, as it is doing this time.
