Confused and anxious by another ESA50 form

I was on IB and DLA since 2005. I was placed in the ESA with Support Group after completing the ESA50 form in October 2011 (without medical). I cannot find anything that indicates/says in the confirmation letter whether there will be a review or not or how long the award will last.
I have now received another ESA50 form from ATOS to complete but the covering letter is worded as though they think I am still on IB and going to complete the ESA50 form for the first time.
The form is due in by 3rd May but every time I took the form out to do it, I became anxious (am on anti-anxiety medication). I wanted to call DWP to say I will not be completing the form as I cannot cope with it but just going to phone makes me anxious as well. I had a letter in March from Jobcentreplus to inform me of the rate change in social security benefits for the coming year.
Do you think they have made a mistake in sending me another form to complete? Should I ignore it? Or do I have to write to ATOS and how should I word the letter? Evelyn

There seems to be a problem with the DWP's standard letter, several members have had a similar one over the last 3-4 weeks, although this may be an error, no one who has received this letter has reported it as such, so you should assume that this is in connection with a re-assessment.
This being the case, you need to complete the form in detail and return it as soon as you are able.
I would recommend that you write to both the DWP and ATOS to tell them that the form will be late and that this is due to your condition(s).
I have to warn you that a failure to return the ESA50, can, but not necessarily will, result in your claim being closed, whilst this is an appealable Decision, it could take months to resolve and you will not be eligible for payment should this occur, so it is really important that you get this form to ATOS as quickly as you can.

I was sent one of these letters with an ESA50 form about 6 weeks ago after winning my tribunal.
In my cases it was sent in error, however i asked Atos why the letter referred to transfer from IB to ESA,
they said to ignore the heading as they were just using up old stationary. Anything to confuse people.
I would however not advise to ignore the form, if you are in any doubt give them a ring as I did.

Just gather all the strength you can and phone Atos, it won't take you more than five minutes to tell them you can't possibly have it back by then.
As for the letter, yes it seems to be a standard use up one which is very confusing when people are already on ESA.
