Perhaps other members will care to comment.
I would agree with you that seeing your normal GP is not the same as meeting with a stranger in an unfamiliar environment.
In terms of evidence the only other thing I can suggest is that you request your medical records.

Whilst doing so may give you access to additional evidence in support of your claim, it will not help with the requirement to attend an assessment.
If you do go, then I would recommend that you take a companion, going on your own will be seen as disproving any evidence that you might have in regard to social anxiety and/or agoraphobia.

There is no reason why a report from an alternate therapist such as acupuncturist should not be accepted as evidence, you should bear in mind that the Decision Maker is not medically trained, so there may be issues on how they will interpret it and whilst they should make efforts not do so, personal biases may influence their Decision.
If they do produce a report, for it to have any weight, it must address the ESA Descriptors that you are being assessed under, so as an example; a report that says you are not fit for work will have little effect, but one that says that you have problems walking, that you would not be expected to be able to manage more than X metres due to pain which is the result of XYZ condition, should be taken into account.

If I am able to find someone, it will not be any of my usual friends who knows me as none of them are able to do the date given which I have stated in the questionnaire.
How would it appear to the assessor if the accompanying person wishes to wait outside in waiting area when I am called in to be assessed, though she would have accompanied me to the assessment centre and into the waiting area?
Or should he/she also go into the assessment ROOM with me? This query is because she herself suffers from mental health problems and would not be comfortable going into the room with me.
However, she is in the position to say something about my mental health as she has witnessed my anxiety state at least once in the last year.
Can she explain to the assessor when I am called that she does not feel comfortable to be in the same room while I am being assessed but will be able to provide something when it comes to the mental health situation?

What are the likely implications of a claimant moving from the place from usual residence AFTER an assessment date was given?
It would be to a different county/area altogether.

From your perspective it would be better if your friend came into the interview room with you, they can offer moral support, take notes, and confirm your statements about your health, but most importantly, providing that this is the case, their presence confirms your need to have someone you trust with you when you meet strangers.
As a partial solution it may be permissible for them to walk you to the room and then leave you there to return to the waiting room, but I can see there being problems with this.
I have to say that I doubt that the assessor will go to the trouble of asking your friend to join you if she does not remain in the room, and is therefore unlikely to ask her about your mental health issues.

What are the likely implications of a claimant moving from the usual place of residence AFTER an assessment date was given?
It would be a move to a different county/area altogether.
Would ATOS set another date in the local centre of the new address or the claimant has to go to the original assessment centre, about 120 miles away?
