I had help from my friend and it took the best part of two days to finish. I have to say the online version is very good and i would recommend it, if at all possible, as it allows you to put in as much information as you want. I would not have been able to write it all into the boxes or have extra sheets of paper.
The only problem is that I was late but I wrote to inform them of the reason for late return, before the deadline, and requested a specified period of time. I then completed the ESA50 form and sent it back within the requested time frame.
All correspdence were posted with next day delivery service. It costs a lot more but it felt 'safer'.
One thing I was not sure about is how to answer the Cancer Treatment question on Page 4.
8 years ago I had cancer of an organ which was partially removed and, within a week of this, it was discovered I also have cancer of the stomach which was treated with chemo tabs. Completely different types of cancer.
Both treatments have been successful and a few months ago, the checkups were extended to yearly (blood tests and/or gastroscopy).
My question: Should I have said yes or no to the question?
Should I have asked my GP to complete page 20 (Cancer Qs) of the new ESA50 form?
I know it is too late now but just want to be 'ready' if I am asked to get the GP to complete page 20.

Well done for getting the ESA50 returned.
Question 4 is in regard to claimants recovering from a recent diagnosis of cancer, as yours was 8 years ago and I assume you in remission and no longer being specifically treated, it would not apply to you.
The next stage of the process is for ATOS to review your ESA50 and evidence to see whether a face to face assessment is required, hopefully one will not be needed, in which case they will return your file to the DWP for a Decision to be made, you cannot be found Fit for Work without having the opportunity to attend a face to face assessment.

Great. That's what I wanted to know.
I am afraid I do not understand this bit you have written 'you cannot be found Fit for Work with having the opportunity to attend a face to face assessment'.
I did write something in the blank page explaining that though the Cancer treatments were successful, the side effects of the treatments (the surgery permanently damaged some nerves causing a great deal of nerve pain and comes without warning) and the chemo resulted in profused sweating, not just perspiration, causing terrible distress and embarrassment.

An ESA assessment can result in three outcomes'
- you are placed in the Support Group
- you are placed in the WRAG
- you are found Fit for Work
You cannot be found Fit for Work unless you have had a face to face assessment with an ATOS "doctor", you can only be placed in one of the two ESA groups.

Just a typo E,
It should have read "you cannot be found Fit for Work without having the opportunity to attend a face to face assessment'.
