New ESA 50 01/13

Hiya one and all.

I think I have managed to complete the latest ESA50 1/13 form to the best of my ability.
Referring to the boxes they ask you to complete, we have put in some information and mention things which relate to the Support descriptors without actually saying so and ask them to read the relevant supporting reports, letters etc. Whether they will or not who knows!!!???
Supporting evidence we have stapled together in each section and numbered accordingly for each section. We have named , NINO each page, sectioned numbered in bottom right corner and top right corner of each page done an overall numbering system.
We have included a superb letter of support from our MP, dated 12/7/13
A full and comprehensive report from our local authority Occupational Therapist for Support Assessment. Dated 23/4/13
A full and comprehensive report from an Independent Occupational Therapist who is a Moving and Manual Handling Trainer and Consultant. Dated 0ct 30th 2012
An excellent report from a Spinal Orthopaedic letter and Dr specialist in MRI scan result . which states will not improve but will deteriorate over time. dated May 8th 2009.
An extremely good letter from GP with supporting evidence. Dated March 2012 (saw him 11/7/13 and he said use old letter explaining he said to use it now, as it was not used last year).
Optical field reports and clinical findings . dated 13/2/13.
Industrial Injury report from DWP stating 50% disabled for life and their findings to support claim. Dated 2006.
Community Health team reports .dated 14/2/12.
Medical Info list providing, lists of medical probs, medicine list again, aids used and supplied from NHS etc.
I have highlighted on the 2 Therapist reports where the Support Group descriptor s apply, without actually saying this is a support group descriptor. Is this ok ?
Now what else would be useful to add, or should we send it off and hope for the best?.
It is not due in until 1st August, but it is all ready to be sent in.
Any help would be much appreciated.

You could actually state which SG Descriptors you feel that these highlighted extracts apply to.

I could say the "kitchen sink", but I won't :P All joking aside, don't be in a rush to send it back, as long as it is posted in good time to arrive by the deadline.

You would be surprised how you pick up on things that you may have missed by reading through things a couple of times. It can also help to get someone else who knows how your limitations affect you to read through it, in case they have any further suggestions.

I wouldn't be afraid of referring to the actual SG Descriptors that you feel apply, and/or ESA Reg 35 if applicable.

You could add comment such as, the list of descriptors supplied that I feel I qualify under, is not exhaustive, and I may qualify under one or more of the other descriptors. Or words to that effect.

You could print off the :

SG (LCWRA)Schedule 3 Descriptors

&

ESA Reg 35 for entry to The SG (LCWRA)

Then highlight and refer to the relevant ones.

It is of course up to you, once you are happy that you have done your best, you can do no more.

Hi I am getting stuff ready for my form now and like you seem to have a lot of paperwork to send hoping this will help me get in to the support group. I have typed a sheet stating the overall effects of all my illnesses and disability and have put at the end.
I hope that from the information on the form, sheets and letters from my medical people you will be able to tell that I am not fit for any sort of work or work related activity.
Don't think it hurts to point them in the right direction!! Sue x

Hiya one and all,

Finally sent off my fully completed ESA 50 1/13 form which I completed as a pdf version and printed it off and added where necessary additional sheets attached.

There were 30 pages of relevant medical reports including Occupational Therapists, GP, Consultants, DLA and Ind Inj for life award sheets and a rather good letter from my MP.

All were attached together, complete with a front sheet providing an index of all pages etc.
All pages numbered for each section and an overall numbering system, full named, D.O.B. and NINO'd.
Held together with a hole punched tag.

Sent via 1st class which will also show proof of delivery and receipt.
( The receipt clearly states it is not a tracked service, so I hope this will be ok)

Now the 'Big Wait'.

I shall keep you updated on results as and when it comes, good bad or ugly!!

I am sure that you have done all you can, so once that you know ATOS have received The ESA50, you can do no more, for now. :)
