Confused and anxious by another ESA50 form

i am in support group for esa and have just recieved one of those forms. the wording confused me at first but as it had been 18 months since my last assessment i was expecting them to get in contact. i am presuming that DWP could not be bothered to write a more accurate form letter and proceeding to fill in esa50 as usual.

I have a medical appointment next week and will not be able to attend as friend cannot come.
I finally called Appointments and asked for another one which would be 6 weeks time. I found out that appointments can only be changed to a later date up to a maximum of THREE weeks from the time you call and only once.
I was advised to call on the day of appoint to make the three weeks further away but this would not help me.
I was advised to get the doctor to send a letter as he/she sent one last time hence I did not get an assessment!
I was informed that if I get my doctor to send a letter, they may not need to see me and if I let them know, they can give time for the doctor to send one.
You bet I am going to 'beg' him to send one. The only thing is I do not know which doctor sent the last one two years ago!
Any ideas how to find out?

In similar scenarios, other member's G.P.'s have Faxed letters to ATOS.
This may be quicker than your G.P. writing a letter and sending it by post.

Just too add,
Any letters previously sent by one of the G.P.'s in your practice, (or any other practice) should be in your G.P. records.
If you have a rough idea when this letter was written, you should be able to obtain a copy of it by requesting this at the G.P. reception.
If it is only one or two pages, they will probably not charge you.

Very gladly told me the name of doctor who wrote the medical report last time and the date it was written with new info that a medical report was requested about 5 weeks ago but they had not received anything, hence an assessment was arranged for me.
My worry now is that I have not seen this doctor for nearly two years. I have called the Surgery but not able to get an appointment anytime in the next three weeks with this doctor.
I have been seeing his colleague regularly in the last two years and now have an appointment to see him the day before the assessment. I am assuming I will have to ask this doctor to write the report.
My question is: can this doctor use the last letter (written by another colleague) and add the conditions that are new since then?
If you think is ok, then I will suggest it to him next week, just to save time.
