Hi Gordon,
Thank you for your response, your advice and help.

It’s embarrassing for me to say but will say it to get your advice if you can...
Q2 I, down to severe anxiety have a problem swallowing, this includes my own saliva. Everything sticks in my throat including liquids, my diet consists of tinned soup, or homemade soup by my mum which is liquidised. Even eating a bowl of soup takes longer than 30 minutes and frequent regurgitation. This is in the presence of my mum or sister, otherwise I wouldn’t eat at all, I would never take soup when they are not in my presence. I cannot do this on my own, my fear of choking is overwhelming. My doctor has this on record, it as mentioned by my psychiatric nurse, it was mentioned in my Mandatory Reconsideration from 2017 by a therapist at the time, it seems if I am not being fed by a tube this doesn’t matter, they stated in my report that weight hasn’t changed, and pardon the pun weighed heavily on that, I weigh 8 stone, on the 12/6/2017 I was 11st 12lb, so I have lost a considerable amount of weight, this is not recorded anywhere medically though.

Q9 I looked at the guide but was rather confused, what I could see on one hand was social support having to be from a professional, on the other point I saw, said it could be from a family member, if you could clarify, that would be great.

Q11. I can’t leave the house without my mother or sister full stop. I cannot go anywhere else.That is just for docs appointments or my psychiatric appointments, if they are unavailable, I cannot attend, my psychiatric nurse even told them I cannot even take my bin out on my own.

Any advice would be gratefully appreciated,

Sorry Gordon,
Meant to put this in previous post. In some of the HP report she refers to a PA3 and another refers to a PA4 , what are these please? Yet stated on the form “list of evidence considered alongside the consultation findings”
AR1 questionnaire dated 04/01/ 2019, which I find bizarre as I only posted it recorded delivery on the 09/01/2019? I have a txt from the DWP stating on Monday the 18th of March 2019 “ thanks for sending us how your disability affects you form. That was just them taking it on board, I have a photocopy of original review form and have a receipt from post office.
They have also included “PIP assesment report 15/12/2016.......this was the HP report on my first ever application, which was refused, on MR I was awarded, so why are they using this wrong information. There would have been another report.

Hard to score points of qu2 for mental problems- they aim more at physical ( unfairly) both taking nutrition and preparing - I've got anorexia and got 2 points for each.
Hard to get f for mobility on mental health- there is a difference between going out and being able to follow a journey of some kind.

Q2 - You would seem to have a good argument for scoring. You say that your problems are recorded but are they the result of a diagnosed condition?

Q9 - The point is that Social Support means specialised input, this is much more than just having someone with you when you are out. So as an example; a relative might provide sign language for a deaf claimant unable to read lips.

Q11 - You are simply confirming that you meet (e). Your ability to follow a route cannot be assessed if you do not go out.

A PA3 and PA4 are assessment reports, the former from a Paper Based Review and the latter from a face to face.

Just because it mentions a PA4 does not mean there is one, the computer system may be adding it automatically and the assessor has not removed it.
