i got fed up with waiting for the call back explanation so chased it, discovered today that it isactually marked as a task to be done by 27th.. Are phone calls from DMs in an office dealing with your claim recorded by the DWP, ie when they phone me to give an explanation.
If not, could I ask permission to record the phone call and if so is it likely they will say yes?
Can I insist on them confirming the 'explanation' comments they make during the phone call be put in writing?

Calls to the DWP should be recorded but I am not sure that those made by them are, you do not need to ask permission to record a telephone call.
You can ask for their comments to be put in writing but I am not sure they have an obligation to do so.

If appealing, and with tribunal - do the dwp, judges etc see your entire medical records?

Only if they are already part of the evidence available or they make a specific request for them, where this happens it is usually the last five years.

Do you think if someone could not hear very well, they would have to put it in writing?

The phone call may defeat that argument but as I said, you can ask.
